Various medical certificates and diagnostic reports The required verification documents vary depending on the applicant. Please check the criteria below before visiting.

  • Certificates are issued upon your visit, so please refer to the non-covered items.
  • If you are an inpatient and need to obtain necessary documents, please inform your assigned nurse or the treatment ward 1 to 2 days before discharge so that you can receive the documents immediately upon discharge.
  • In accordance with Article 21 of the Medical Service Act, since the cost of copies of medical records can be legally charged to the patient, only the person subject to issuance or their legal representative may request all documents and imaging data (X-ray, CT, MRI, etc.).
  • To request a copy of medical records, you must verify the applicant's identification. The patient or their representative is requested to visit with their identification and required documents.

Notice

  • The ID card or a copy of the ID card must have a photograph attached.
  • For patients aged 14 to 16 who are not of the age for issuing a Resident Registration Card, a photographic document (such as a student ID) to verify their identity may be requested. However, this is not a mandatory regulation.
  • The certificate of kinship must show the relationship with the patient.
  • Signatures on the consent form and power of attorney may be handwritten; seals and fingerprints are not accepted.
  • The 'Access to and Issuance of Copies (Scope)' in the consent form must be specifically stated, including the diagnosis, treatment period, and personal expenses.
